What Are the Side Effects of GLP-1 Weight-Loss Medications?

The most common side effects of GLP-1 medications like semaglutide and tirzepatide are digestive: nausea, constipation, diarrhea, and sometimes reflux. They tend to be worst during dose increases and ease as your body adjusts. Most are mild to moderate and manageable with a slow dose ramp and simple adjustments. Serious side effects are uncommon, but a few are worth knowing about.

In my more than 20 years in medicine, I have learned that patients do far better when they know what to expect, so here is the honest rundown, not a scary list and not a sugarcoated one.

The everyday side effects are almost all in the gut. Semaglutide tends toward nausea and constipation, while tirzepatide more often causes diarrhea with less nausea. These usually show up when the dose is increased and settle within a week or two. The good news is that most of them are very manageable.

In my practice, here is what I see. The patients who struggle with side effects are usually the ones who were pushed up in dose too fast, or who were left on their own to figure it out. Done right, with a slow ramp and real monitoring, most people tolerate these medications well and stay on track.
